What is a patient service plan?Garvin Dentistry offers an in-house Dental Service Plan for individuals without traditional dental insurance. We understand that traditional dental insurance premiums can be expensive while often not covering the best treatment for patients. We also understand the importance of affordable dentistry. In an effort to help provide basic care at a controlled cost and offer a discount for all other dental treatment, we present our service plan. The Garvin dentistry service plan offers your preventive care treatment at no cost. This includes 2 healthy cleanings (in the absence of periodontal disease), exams, and x-rays per enrollment year. Your plan also provides you with 10-20% off on all other treatments offered by Garvin Dentistry. (Some restrictions may apply)

PLAN ENROLLMENT
Eligibility and Restrictions
• The Garvin Dentistry Service Plan is not dental insurance• The Garvin Dentistry Service Plan cannot be used in conjunction with any other discount offers or insurance plans
• Participants become eligible upon payment in full for one year from the date of enrollment
• Eligibility ends when membership period ends
• Eligible Family Members include those living in the same household, spouse or dependents
• Membership must be paid in full at the time of enrollment
• Treatment fees are due and payable at the time service is rendered
• Enrolling in the Garvin Dentistry Service Plan will allow you the opportunity to obtain dental treatment at reduced prices
• Treatment is limited to procedures within the scope of Dr. Garvin and his staff even if treatment outside of that scope is recommended
